STEP ONE:

1.1   Fill out the Registration Form (FR001-C)

1.2   Submit the photocopy of listed requirements at the College Registrar’s Office.

1.3   Take the MHCC Entrance Exam on the scheduled date.

       *Free Entrance Examination

1.4   Exam results will be announced on the day of the interview.

 

STEP TWO:

2.1   After receiving the Exam result, the applicants will be scheduled for the admission interview with the College Dean.

2.2   After the admission interview, the Dean shall endorse the applicant to the College Registrar’s Office for enrollment.

2.3   The applicant fills out the Admission Form (FR002-C).

 

STEP THREE:

3.1   The qualified applicant must submit the original copies of their requirements.

3.2   Fill out the Subject Evaluation Form (FR004-C) for the subjects and units to be taken during the semester.

3.3   The College Dean evaluates the subjects and signs the Subject Evaluation form.

 

STEP FOUR:

4.4   The College Dean returns the Subject Evaluation Form to the   Office of the Registrar for enlistment and to secures three (3)   copies of an endorsement for assessment (FR003-C) and payment for tuition, miscellaneous, and other fees.

4.5   Proceed and submit the (3) copies of the Assessment Form to the Cashier’s Office for payment.

4.6   Get FR003-C Student’s Copy and FR003-C Registrar’s Copy.

 

STEP FIVE:

 Present to the College Registrar’s Office the official receipt and FR003-C for the “ENROLLED” stamp after payment.

 

Registration by the representative will be allowed during enrolment.

Kindly present the following:

1. Original and photocopy of MHCC ID/ any ID for the old and new student with picture and signature of the student or applicant.

2. Authorization Letter with Signature.

3. Original and photocopy of any ID with picture and signature of the representative.
